CareFocus

CareFocus provides support services to enhance the quality of life for persons with a range of mental health and developmental disabilities. Our programs promote personal independence and assists with meeting and exceeding the individual goals of our service recipients. We empower our recipients to discover their own way in the world - teaching them how to think, evaluate, and decide for themselves.

At CareFocus, we also understand that our service recipients are of varying ages and have multifaceted disabilities. With this in mind, our support plans are developed to meet the needs of adults and youth, empowering them to drive the progress of their own growth. We offer services to help manage various behavioral challenges and developmental disabilities, focusing on the service recipient’s abilities, rather than his or her disabilities.

CareFocus offers flexible and reliable services 24-hours a day, 7 days a week, assuring that our service recipients always have the support they need.

Our results-driven approach is designed to involve the service recipient’s family, beginning with our Person-Centered approach, and continuing through our developmental, behavioral, and socialization support services. Strengths, as well as undeveloped skill sets, will be highlighted and interventions will be measured to ensure the highest quality of support is provided. Our approach offers individualized strategies to achieve optimum inclusion in our communities, family, and support providers’ network.

CareFocus Living Programs

CareFocus also offers a variety of residential and support programs, allowing our service recipients to receive the care they need in an environment that it is most convenient for them. These services include:

Residential and Community

• Supervised, therapeutic home services that allow the recipient to receive the 24-hour therapy and support they may need
• Companion services on a one-on-one basis where 24-hour support may be necessary in a recipient’s home
• Securing and maintaining employment
• Periodic residential support visits to a recipient’s home to ensure safety, well-being, and personal growth
• Daily living support
• Behavioral and personal life guidance
• Developing community connections and relationships
• Coordination of clinical needs
• Crisis stabilization
• 24-hour or periodic support to families and their children with behavioral and/or emotional challenges
