Process and Assessment

Concentrating on the individual abilities of our service recipients allows CareFocus to assist with establishing a complete personal growth path. Our initial goal is to develop a program that enhances the overall quality of life and involves not only the service recipient, but also family members, CareFocus mentors, and other professional staff.

Assessment

As part of the initial assessment, CareFocus will meet with the service recipient, his or her family, and caregivers to gain insight into the dynamics of each relationship in order to create a personalized plan for success. During this initial meeting, the recipient acts as a team leader, providing input for a plan that will focus on promoting personal growth and development.

CareFocus strives to maintain and assure an adequate, on-going quality assessment and evaluation of program activities and procedures involving all members of the team. Each member of the team contributes to the service recipient’s achievement and progress, providing guidance and support.

Program Evaluation

CareFocus will actively evaluate each service recipient to ensure they are receiving the highest level of program quality, effectiveness, and efficiency. Progress is monitored and reviewed through survey evaluations and Personal Outcome* interviews given to the service recipient, families, and caregivers. Surveys ensure we are receiving the most accurate and concise information possible, and are one of the many tools that we use for evaluating progress. Continual program evaluation is imperative to the success of the recipient, and offers the family insight into the abilities and potential of the recipient.
